Sense-making

When conducting a problem identification exercise, don’t start with the solution, identify the problem first.  Facilitated sense making exercisers involving stakeholders will help highlight and created a shared understanding of the real problems that exist within the workplace. A shared understanding of the complete socio technical system leads to solutions that are more acceptable to end users and deliver better value to stakeholders.

Root cause analysis

A Root Cause Analysis is a reactive way of identifying and solving problems. With experience its use can change from reactive to proactive, so that problems can be anticipated before potential harm occurs. Corrective steps to eliminate an identified problem are driven through a process of compliance.
